The title is a reference to "Thy Flesh Consumed," the original fourth episode which the WAD replaces. The WAD is sometimes considered an extension of ConC.E.R.N.ed, Powell's earlier Ultimate Doom three-episode megawad, though this has never been properly confirmed. The episode is intended to be difficult, in keeping with the original episode 4.
